In addition to our commitment to provide GMEI trainees with an exceptional educational experience, we are committed to providing a benefit package that will protect the health and welfare of our valued trainees – you. Instructions are provided for each of the benefits listed below. Please take the time to review each benefit, complete the required forms, and either mail the forms to GMEI or bring them to your orientation. Completing this information prior to the start of your training program will ease your transition and prevent any delays in receiving your paycheck or enrolling in the benefit programs.
Social Security Number
In order to work in the United States, you must have a social security number (click here for application assistance). If you do not already have a social security number, please apply for one today so as not to delay your stipend check once you start your training program.
Michigan Driver’s License
Michigan Department of State determines the application requirements for new Michigan residents older than 18 years of age. Application for a driver’s license requires proof of residency and identification. The terms are described clearly on the Michigan Department of State website.
Direct Deposit
GMEI requires direct deposit of trainee paychecks. Please send a voided check (simply write VOID across the front of the check) to the Operations Manager to ensure the account and routing numbers are recorded accurately. If you do not have a checking account, local banks you could consider include National City and MSU Federal Credit Union. For branch location and checking account applications, please visit the websites below.
Tax Forms
Completed forms for federal, state and local taxes are required. In addition to the forms the Federal Government requires the I-9 form which calls for evidence of identity and eligibility for employment and the State of Michigan requires a New Hire From. While instructions are often provided, you may want to consult your tax preparer prior to completing these forms.

All trainees, citizens and noncitizens, hired after November 6, 1986, must complete Section 1 of this form at the time of hire, which is the actual beginning of training. GMEI staff must examine evidence of identity and eligibility for training within 3 business days of your start date. Please make sure you bring the necessary evidence to your program’s orientation to ensure no delay in starting.

Please note that the total number of exemptions you claim on the MI-W4 may not exceed the number of exemptions you are entitled to claim when you file your Michigan individual income tax return. If you are married and you and your spouse are both receiving stipends while in training or employed, you both may not claim the same exemptions. Typically, the exemptions calculated for the federal W-4 form are the same for the MI W-4 form.
- Michigan New Hire Form and Instructions
Federal legislation, effective October 1, 1997, requires all Michigan employers, both public and private, to report all newly hired, rehired, or returning to work employees to the State of Michigan. As a Graduate Medical Education trainee receiving a stipend from GMEI we must report this relationship to the State.

Health Care Insurance
GMEI provides health insurance through Physician Health Plan of Mid-Michigan. Our plan is called PHP Plus. GMEI pays 100% of the premium (Click here for a brief summary of the benefits). For more information and to review the provider network please refer to the PHP of Mid-Michigan website. Please note that our program does not limit care to specialists or in network providers.
In order to enroll you and your family in the benefit, you must complete sections A-G on the enrollment form.
Dental Insurance
GMEI provides dental insurance through Delta Dental of Michigan. GMEI pays 100% of the premium (Click here for a brief summary of the benefits). For more information on Delta Dental and to review the network of participating dentists please refer to the Delta Dental website.
In order to enroll you and your family in the benefit, you must complete the enrollment form.
Life Insurance
GMEI provide life insurance through Symetra Financial. GMEI pays 100% of the premium (Click here for a brief summary of the benefits). In order to enroll you and your family in the benefit, you must complete the enrollment form.
Long-Term Disability Coverage
GMEI provides long-term disability insurance through Provident Life and Accident Insurance. GMEI pays 100% of the premium (Click here for a brief summary of the benefits). In order to enroll you and your family in the benefit, you must complete the enrollment form.
Worker’s Compensation Coverage
All trainees are protected by workers’ compensation insurance through MHA Service Corps. GMEI pays 100% of the premium. The policy covers injury or illness resulting from legitimate training activities (such as needle sticks). By law, GMEI is required to report injuries covered under workers’ compensation within twenty-four (24) hours. Please report any work-related injury or illness immediately to your supervisor and/or Executive Director. Notice of Injury forms are available here and should be faxed to GMEI at (517) 432-5694.
Additional Benefits Available to Graduate Medical Education Trainees
As a trainee in an accredited graduate medical education program, certain additional benefits are provided to support the educational process. Please consult your program’s training manual for the specific list of benefits and any associated limitations.
